A scarification is essentially a tattoo without the ink portion of tattooing or the modern machinery usually involved. Scarifications are usually performed with a sharp blade or scalpel and the skin is cut, hatched or removed in pieces to create a pattern or design. Here’s the Wikipedia page on them that explains some of the reasons people might get them and the traditional importance behind them.

My own scarification was done in a sterilized environment with a surgical scalpel(s). My wife (who is an artist, knows several important tattoo/body modification artists and consulted with them, and has taken health and sanitation courses - technically a nursing class - and previously performed piercings) did the entire process from design to layout to the actual cutting and surgical removal of some areas of my skin (the 3rd time we did a scarification). She’s definitely not squeamish and has a very steady, flowing hand.
